Eligibility information for Mass General Brigham SCO HMO-DSNP and One Care HMO-DSNP

You can enroll in one of our plans if you:

Are eligible for MassHealth and Medicare

You must be eligible for Medicare to enroll in one of our plans.

You must also be eligible for MassHealth (Medicaid). The requirements are different for SCO (Senior Care Options) and One Care:

For Mass General Brigham SCO, you must be eligible for MassHealth Standard.
For Mass General Brigham One Care, you must be eligible for MassHealth Standard or MassHealth CommonHealth.

Meet the age requirements for the plan

There are different age requirements for SCO and One Care:

For Mass General Brigham SCO, you must be ages 65+.
For Mass General Brigham One Care, you must be ages 21 to 64.

Live in one of these Massachusetts counties

Bristol, Dukes, Essex, Middlesex, Nantucket, Norfolk, Plymouth, Suffolk

Have no other health insurance plan

You cannot be enrolled into any other comprehensive health insurance plan (except Medicare).

If you're ready to enroll in Mass General Brigham SCO or One Care, you can connect with a Plan Associate. A member of the team will help you review plan benefits and determine your eligibility. If you are eligible, they can help you complete an application by telephone.

Mass General Brigham Health Plan is an HMO-POS/PPO organization with a Medicare contract. Enrollment in Mass General Brigham Health Plan depends on contract renewal. Other providers/pharmacies are included in our network.

Mass General Brigham One Care is a Dual Special Needs Plan (D-SNP) that contracts with both Medicare and MassHealth (Medicaid) to provide benefits of both programs to enrollees. Enrollment in the plan depends on the plan’s contract renewal with Medicare. This plan is a voluntary program that is available to anyone age 21 - 64 who qualifies for MassHealth Standard or CommonHealth and Original Medicare and does not have any other comprehensive health insurance, except Medicare.
