About our plans
Mass General Brigham SCO and One Care plans include support from a care coordinator and an integrated care team. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental, vision, and hearing care, as well as transportation to your appointments.
SCO is a health plan for people ages 65+ who have MassHealth (Medicaid) and Medicare. One Care is a health plan for people ages 21 to 64 who have MassHealth (Medicaid) and Medicare. If you are turning 65 and are already on a One Care plan, you can stay on the plan.

Mass General Brigham One Care is a Dual Special Needs Plan (D-SNP) that contracts with both Medicare and MassHealth (Medicaid) to provide benefits of both programs to enrollees. Enrollment in the plan depends on the plan’s contract renewal with Medicare. This plan is a voluntary program that is available to anyone age 21 - 64 who qualifies for MassHealth Standard or CommonHealth and Original Medicare and does not have any other comprehensive health insurance, except Medicare.
