About your plan

Mass General Brigham SCO and One Care plans include support from a care coordinator and an integrated care team. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental, vision, and hearing care, as well as transportation to your appointments.

SCO is a health plan for people ages 65+ who have MassHealth (Medicaid) and Medicare. One Care is a health plan for people ages 21 to 64 who have MassHealth (Medicaid) and Medicare.

Get started by creating an account on the Member Portal

The Mass General Brigham Health Plan Member Portal is a convenient way to access important plan documents and benefit information. With a portal account, you can:

  • Access a digital version of your ID card
  • View your plan information
  • Search for providers and hospitals
  • Review and submit reimbursements

You can also download the Mass General Brigham Health Plan Member app for Android and iOS to access these features on the go. If you prefer not to use the online portal or app, you can still access these services by calling Member Services or requesting printed materials.

Frequently asked questions

Below are a few of the most common questions we hear from new members.

After you enroll, you will receive a letter from MassHealth confirming your enrollment. You will also receive new member materials including a member ID card and benefit guide. A care team member will reach out with a welcome call to schedule your first health assessment.

Once your benefits are active, you can register for an account on our Member Portal. There will you find useful resources and tools like your Member Handbook, otherwise known as the Evidence of Coverage. You can also find member resources to help you use your plan and benefits on our Mass General Brigham SCO and One Care forms and documents page

When you enroll, we will assign a care coordinator to help you get the care you need. This person will work with you, your primary care provider (PCP), and your care team to create a personalized plan.

If you need more support, your care team might also have:

  • A services coordinator who can help you find resources in your community
  • A pharmacist who knows your prescription medications
  • Specialists and other health care providers for your health conditions
  • Social, behavioral, and other specialized supports

As an extension of your care team, you can also call our One Care 24/7 Nurse Advice Line at 833-551-3255 (TTY 711) or our SCO 24/7 Nurse Advice Line at 833-511-3252 (TTY 711) to speak with a registered nurse about any of your healthcare concerns.
