Mass General Brigham SCO and One Care plans include support from a care coordinator and an integrated care team. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental, vision, and hearing care, as well as transportation to your appointments.
SCO is a health plan for people ages 65+ who have MassHealth (Medicaid) and Medicare. One Care is a health plan for people ages 21 to 64 who have MassHealth (Medicaid) and Medicare.

Below are a few of the most common questions we hear from new members.
After you enroll, you will receive a letter from MassHealth confirming your enrollment. You will also receive new member materials including a member ID card and benefit guide. A care team member will reach out with a welcome call to schedule your first health assessment.
Once your benefits are active, you can register for an account on our Member Portal. There will you find useful resources and tools like your Member Handbook, otherwise known as the Evidence of Coverage. You can also find member resources to help you use your plan and benefits on our Mass General Brigham SCO and One Care forms and documents page.
When you enroll, we will assign a care coordinator to help you get the care you need. This person will work with you, your primary care provider (PCP), and your care team to create a personalized plan.

You'll pay $0 for prescription drugs and over-the-counter products on our One Care plan and SCO plan drug lists. You can check the drug list, also known as a formulary, on our SCO and One Care Pharmacy Benefits page.
Dental – Our SCO plan includes:
Vision – Annual eye exams are $0 for members. You’ll also get $200 a year to buy glasses and contact lenses.

Vision – Vision services, including exams and glasses, are covered every two years. Medically necessary contacts may be approved by your provider in place of glasses.

My Ombudsman is a free, independent program for all MassHealth members. It helps resolve conflicts and questions about MassHealth benefits and services. My Ombudsman will work with you to make sure you receive your benefits and exercise your rights within your health plan.

Emergency transportation -- Emergency transportation is covered.
Medical transportation – Our One Care and SCO plans include unlimited rides to medical appointments. Please call CTS at 844-247-2100 (TTY:711) to schedule your appointments.
Non-medical transportation – Non-emergent non-medical transportation includes two (2) round trips or four (4) one-way trips per month with a 50-mile limit each way. Please call CTS at 844-247-2100 (TTY:711) to schedule your appointments.

Your care coordinator can help you access resources to help with your health and well-being. You may be eligible for long-term support services that provide hands-on care and monitoring in your home, allowing you to live independently and get help with everyday activities like getting dressed, eating, taking a bath, and laundry.
