What are SCO and One Care plans?
SCO and One care plans combine MassHealth and Medicare benefits. These plans give you extra help and care when you need it.
The Mass General Brigham Health Plan SCO and One Care plans come with support from a care coordinator and an integrated care team. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental and vision care, and transportation to your appointments.
Mass General Brigham SCO is a health plan for people ages 65+. This plan supports independent living and includes a fitness benefit to help you say active.
Mass General Brigham One Care is a health plan for people ages 21 to 64. This plan includes support for behavioral health and long term care to help you live independently in your community.

Enrollment in the plan depends on the plan's contract renewal with Medicare. This plan is a voluntary program that is available to anyone age 21 - 64 who qualifies for MassHealth Standard or CommonHealth and Original Medicare and does not have any other comprehensive health insurance, except Medicare.
