Mass General Brigham One Care is a health plan for people ages 21 to 64 who have MassHealth and Medicare. It’s easy to use because it combines the benefits you already have into one health plan with one card.
It also includes support from a care coordinator and an integrated care team. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental and vision care, and transportation to your appointments.

MassHealth offers different types of coverage based on your age, health status, and living situation. You can visit mass.gov to apply for MassHealth coverage. After you apply, MassHealth will determine which program or coverage type you can get. If you are selected for MassHealth Standard or MassHealth CommonHealth, you may be eligible for Mass General Brigham One Care.
If you're getting Social Security disability benefits, you'll get Medicare automatically after getting disability benefits for 24 months. If you have ALS (also called Lou Gehrig’s disease) you’ll get Medicare automatically as soon as you start getting disability benefits. If you have end-stage renal disease, you are eligible for Medicare if you meet the requirements listed here.
If you are eligible for Medicare Part A and Part B, you may be eligible for Mass General Brigham One Care.
When you enroll, we will assign a care coordinator to help you get the care you need. This person will work with you, your primary care provider, and your care team. Together, they will make a care plan for your personal health needs.

Vision – Vision services, including exams and glasses, are covered every two years. Medically necessary contacts may be approved by your provider in place of glasses.

Hearing – Coverage includes hearing exams and hearing aids.
Medical transportation – Our One Care plan includes unlimited rides to medical appointments.
Non-medical transportation - Non-emergent non-medical transportation has a benefit limit of 2 round trips per month, and a 50 mile limit each way.
Personal help programs – You may be eligible for long term support services (LTSS). These services provide hands-on care in your home. You can get help with activities like getting dressed, eating, taking a bath, and doing laundry.
